OVERVIEW

Southside Collaborative is hiring a Community Health Worker to help grow our health clubs and lead outreach efforts across San Antonio’s South Side.

KEY RESPONSIBILITIES

 

Health Club Coordination

  • Organize and help manage weekly health club meetings, including logistics, curriculum and communication with participants.

  • Support participant leadership development to encourage active community involvement.

  • Ensure activities are relevant, interactive and aligned with the Southside Collaborative’s mission.

 

Community Outreach

  • Help plan and staff outreach events across San Antonio’s South Side, engaging residents in health and wellness initiatives.

  • Assist in follow-up communications with individuals and organizations met through outreach.

  • Work alongside the Program Manager to grow the Collaborative’s presence and reach.

 

Partner Engagement

  • Coordinate with community partners to support health clubs and outreach activities.

  • Maintain an up-to-date list of community partners and upcoming events.

  • Support outreach efforts to deepen and expand partner relationships.

 

Communications

  • Clearly communicate the mission and goals of Southside Collaborative to community members, program participants and partner organizations.

  • Use social media, texting, and digital tools to promote programs and increase engagement.

  • Assist with regular program updates and coalition meeting coordination.

 

REQUIRED QUALIFICATIONS

  • 2+ years relevant experience in community development, education, social services or health care

  • High school diploma/GED or college degree in a related field

  • Significant lived experience with community organizing around health and wellness

  • Community Health Worker (CHW) certification (or in progress)

  • Knowledge of Social Determinants of Health

  • Project management experience

  • Proficiency in MS Office.

  • Bilingual (English & Spanish) — to include speaking, writing and reading

  • Valid Class C Texas Driver’s License.

 

ESSENTIAL SKILLS & KNOWLEDGE

  • Self-motivated and able to work independently or as part of a team.

  • Strong project management skills, including the ability to handle scope, requirements, deliverables, timelines, and resources across complex, multi-stakeholder efforts.

  • Skilled at facilitating diverse groups to reach shared goals.

  • Excellent oral and written communication skills. Comfortable communicating with a wide range of audiences, including:

    • Community-based organizations

    • Government agencies

    • Philanthropic foundations

    • Researchers and educators

    • Health professionals

    • Service providers

 

OTHER REQUIREMENTS

  • Must have daily vehicle access with current registration and insurance that complies with Texas law.

  • Ability to lift/carry 25 lbs. and work in various settings.

  • Available for evening/weekend events with notice.
